Insurance
The following is an attempt to help reduce the confusion of the alphabet soup of insurance plans.

PPO = Preferred Provider Organization
Normally no referral is necessary and usually a co-pay is paid to the specialist at each visit. Often a percentage is also due. For this latter charge, you can pay under our Easy Pay Plan.
HMO = Health Management Organization
A written referral is always required to see a specialist. This must be obtained from a Primary Care Physician (PCP) before seeing a specialist. Usually a co-pay is paid at each visit. Occasionally a percentage is also due after insurance has paid the physician.
* Dr. Rollins is not a member of any HMO plan.
POS = Point of Service Plan.
There are no standard rules for these plans. They vary from insurance company to insurance company. It is best to check with your insurance company's Member Services Rep. Often these plans have "Tiers" involving in-network and out-of-network coverages. The different tiers effect the amount of your co-pay responsibility.
* Dr. Rollins is considered an out-of-network provider for Great West and HealthNet plans.
EPO = Exclusive Provider Organization (very similar to POS plans)
MC = Managed Care
These are different from an HMO in that usually only a verbal referral is necessary. Usually a co-pay is paid to the specialist at each visit. Sometimes a percentage is also due. For this latter charge, you can pay under our Easy Pay Plan.
Liability Plans
These are normally 80-20 or 70-30 plans and may also involve a co-pay collected at the time of visit. For the % charge, you can pay with our Easy Pay Plan.
Each of the above plans may include an annual deductible. It is your responsibility to know the terms of your insurance coverage.
Dr. Rollins is a member of most insurances except as noted above.
