From 880 - Take the Lark Avenue exit, making a left turn at the first light, onto Lark Avenue. At the T-intersection, turn left onto Los Gatos Boulevard. At the next traffic light, Samaritan Drive, turn right and proceed to the 2505 Samaritan Drive complex on the left. Take the third driveway entrance, right before the hospital bus stop. Park immediately. Suite 209 is in the first set of buildings, first door on the right.
From HWY 85 - Take Bascom / Los Gatos Blvd. exit, turn south onto Los Gatos Blvd., then turn left onto Samaritan Drive and proceed to the 2505 Samaritan Drive complex on the left. From Santa Cruz - From Highway 17 North, take the Lark Avenue exit, merging right onto Lark Avenue. At the T-intersection, turn left onto Los Gatos Boulevard. When the office is closed, an exchange service takes calls and relays medical emergencies to the doctor on call. Requests for prescription refills are NOT approved during non-office hours.
